Payment of my standard fee if required at the conclusion of each session. The issue of insurance coverage has become quite complicated. If you choose to seek reimbursement for services from your insurance carrier, I will assist in providing receipts of payments to you. The responsibility is yours to be sure that the coverage is in effect and that the proper procedures (i.e. pre-approval, limits, deductibles etc.) are followed for services to be covered. Please understand that your insurance benefits, if any, result from an agreement between your insurance provider, your employer, and yourself.  I am not a party to this agreement, nor do I have any control over its terms. I will notify you in advance of any changes in my policies and fees.
